The office hours for individual practices and ancillary departments do vary according to the needs of the patients. Each practice has a direct phone number. For better service and office hours, you are encouraged to call your provider’s office directly. The number for after hours and emergency calls can be obtained on a recording from your provider practice. Calls regarding appointments and prescription refills should be handled during regular office hours.
In order to provide the best care for our patients, appointments are required for all office visits. To schedule a visit with your provider, call that office directly. As a courtesy to other patients, please give a 24-hour notice to our provider if you are unable to keep your scheduled appointment. Please keep in mind that there is a no-show charge for appointments that are not cancelled in advance.
Upon Your First Visit
If this if your first visit to your provider’s office, you should arrive at least 15 minutes prior to your scheduled appointment. This will allow for the completion of necessary registration information.
Medical records are kept according to Ohio laws. NorthWest Ohio Primary Care Physicians, Inc. maintains an electronic medical record for all of their patients. Medical records are strictly confidential. You records cannot be released to someone other than your provider without your written permission.
About Your Medical Bills
Your monthly statement encompasses professional services provided by the NorthWest Ohio Primary Care Physicians, Inc. If you are hospitalized, you will be billed separately by the hospital for room and other related charges. As a convenience to our patients, a staff member will be available to answer your questions regarding your account balance at 419-931-3081.
You do not need to file a claim with your insurance carrier. NorthWest Ohio Primary Care Physicians, Inc. will submit all claims as a service to our patients. Your insurance policy is a contract between you and your insurance company. It is important that you understand its provisions. Our providers contract with various provider organizations and we will honor those agreements. In those situations, appropriate contractual adjustments will be made following receipt of payments.
All patients who do not have insurance are asked to pay for their services on the day of the visit. NWOPCP, Inc. requires a minimum payment of $25.00 each month on a balance of less than $100.00. A balance larger that $100.00 requires a monthly payment of at least one-fourth (25%) of the original charges.
MyChart Patient Portal
NWOPCP, Inc. utilizes a patient portal system called MyChart. This is a great way to become engaged in your health care! You can request or change an appointment, refill a prescription, ask your provider a question and review your health record. Please note requests submitted on MyChart will be addressed by the end of the next business day and are not addressed on the holidays or weekends. Want to join MyChart?
